Standard Application

• Pay stub(s) for current month
• W-2 forms for past 2 years
• Federal tax returns (all schedules) for past 2 years (if self-employed or commission income)
• Corporate tax returns for last 2 years (if self-employed)
• Bank statements on all accounts (all pages), 2 months
• Statements on investment accounts (all pages), 2 months
• Gift letter (if applicable)
• Divorce decree with property settlement (if applicable)
• Sales contract (for purchase only)
• Name, account #, and phone # of landlord (covering 12 months)
• Application fee of $380
• Personal Detail Sheet (optional)

Military Application

• Copy of DD214 and Original Certificate of Eligibility
• LES statement covering the most recent 30 days
• Housing Card and Statement of Service from Commander
